Mowry Commons Field Trip Information Sheet & Waiver
Dear Parent or Guardian,
Revive the Roots and 15 Minute Field Trips have teamed up to provide field trips to Mowry Commons in Smithfield, Rhode Island. On this field trip your child will participate in activities that foster curiosity and learning about planet Earth and the communities of insects, animals and plants that call it home. Please prepare your child to be outside for the 2 hour field trip. We have a covered pavilion space, running cold water and port-a-potty but no heated spaces. Please make sure your child is dressed appropriately for the weather including shoes. Paths may be muddy & wet during your trip. We strongly encourage waterproof footwear for all visitors. Please DO NOT send your child to school in SANDALS on the day of the field trip.
It is required that you sign this document for your child to attend this trip. Please read thoroughly and complete the signature section before the scheduled trip.
Where: Mowry Commons 374 Farnum Pike, Smithfield RI. Children spend the majority of their field trip time in the community gardens and wooded trails.
Information for Chaperones & Teachers: This is a field trip for chaperones who love to help out! Chaperones & Teachers should be ready to participate and help with assigned tasks such as helping students at the handwashing station.
Safety: Revive the Roots and 15 Minute Field trips are trained to provide safe and educational programs for all ages. Each instructor carries a small first aid kit in case of small injuries associated with outdoor activities. Bug spray and sunscreen is available on site. No children are allowed in the sheep paddock, bird coops or beekeeping areas for their safety.

Field Trip Waiver and Signature Page
I the Parent/Guardian, acknowledge and understand that there are certain risks associated with my child (the participant) on an outdoor field trip (Activity) to Mowry Commons (Premises). I acknowledge that the Activity will involve exposure to natural environments and other elements and activities commonly associated with the outdoors. I further acknowledge that various agricultural and related activities are conducted on the Premises, including farming of livestock (sheep, chickens and ducks) and beekeeping. I acknowledge that these and other elements associated with the Premises and the Activity include but are not limited to risk of: trips, falls, scrap and/or bug bites. I understand that my child is participating voluntarily and I have been made aware of the associated risks. Additionally, Revive the Roots (RTR) has been made aware of any conditions pertaining to the child that would increase the likelihood of experiencing injuries while engaging in certain activities on the premises.
Hold Harmless: I also agree to indemnify and hold RTR and 15 Minute Field Trips harmless from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees, arising from the Participant’s involvement in The Field Trip, and to reimburse it for any such expenses incurred.
Consent to Medical Treatment: I hereby consent to Participant’s receipt of medical treatment deemed necessary if Participant is injured on the Premises or requires medical attention during Participant’s participation in the field trip. I understand and agree that I am solely responsible for all costs related to such medical treatment and any related medical transportation. I hereby release, forever discharge, and hold harmless RTR and 15 Minute Field Trip from any cost or claim based on such treatment or other medical services.
Severability: I further agree that this Waiver of Liability, Assumption of Risk, and Indemnity Agreement is intended to be as broad and inclusive as permitted by law, and that if any portion is held invalid the remaining portions will continue to have full legal force and effect.
Governing Law and Jurisdiction: This Agreement shall be governed by the laws of the State of Rhode Island, and any disputes arising out of or in connection with this Agreement shall be under the exclusive jurisdiction of the Courts of the State of Rhode Island.
Acknowledgment of Understanding: I have read this Waiver of Liability, Assumption of Risk, and Indemnity Agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I confirm that I am signing the agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
